Renewing a Food License using ezTrak
Renewing an FSO or RFE License
Beginning with the 2024 license renewal cycle, the City of Cincinnati Health Department will require all food license holders to utilize the CAGIS ezTrak online platform for license renewals. Payments options on this site include credit card and e-check. After creating an account in ezTrak, you will be able to:
- View, renew, and pay for ALL your food facility license(s) online
- Update the Primary Contact and License Holder contact information
- View Health inspection reports (coming soon)

There’s also a related video about "Managing Relationships with Delegation" - this will help you set up other ezTrak users that may access your records to perform actions related to your licenses.

Using ezTrak
Once you have an ezTrak account, hit "Login" at the top of the window and login with your ezTrak account. After you login, you'll be on the Action Center page.
- Action Center page on ezTrak- The Action Center is your landing page in ezTrak. It shows records and licenses that you need to take action on (pay fees, renew, etc.). You can also get to the Action Center by going to the top of the page and selecting "Records" and then "Action Center".
- My Records page on ezTrak - You can see all of your records by going to the top of the page and selecting "Records" and then "My Records". There is a summary of your records followed by a section called "My Record Details" and "All Records" that displays links to all of your records including links to any food safety license records that you have.
- You will see your license records on the Action Center “Renewals Pending” tab. To renew a license, select "Renew".
- Updating Contact Information in ezTrak - All contact information can be updated through initial creation of your ezTrak account and can also be continuously updated at a later date through ezTrak. Each food license record must have full contact information for the license holder entity and a primary contact added as part of the account creation process. The primary contact is the entity responsible for all official notifications regarding the license record and its management.
Food licenses are NOT transferable from one individual or entity to another. If you have recently purchased the food facility, and you are not the owner listed on the current paper food license you hold, you may not renew the license through this process - a new license is needed.
