• Non-refundable $3 search fee for certificates not found.
• Birth certificates are available from 1909 to present.

 

We require the following information to complete your order:

• Full name as listed on the birth certificate.
• Date of birth.
• Mother's name and maiden name (mother's name prior to first marriage).

• Certificates not picked up by 3:45 p.m. will be placed in the mail. 
• We are not responsible for any postal delays, nor will a certificate be replaced if lost/stolen/damaged during the delivery process. 
• Certificates mailed can take up to 20 business days for delivery.
