All benefits enrollment, whether new hire, promotional, qualifying life event changes, or open enrollment, will take place online at
www.COCBenefits.com or by calling 1-866-477-1604.
The New Employee Packet is available here and should be provided to employees electronically prior to their start date. You can also download a print version of the packet, which includes all benefit required notices, for employees who will not have immediate access to email.

Please remember, employees have 30 days from their date of hire to complete their benefits enrollment. Benefits enrollment is also contingent upon the department promptly entering new employees into CHRIS. Thank you for your cooperation!
Medical Provider (Health & Pharmacy)
Anthem is the City's medical AND pharmacy provider for both the 80/20 Plan and the HDHP Plan. Login to Anthem.com for information on network providers, claims status, and other questions you may have. You can also download the Anthem Sydney Health App for iPhone or Android to have mobile access to your cards and records at all times.

Superior Dental Care is the dental provider for CODE, Building Trades, Fire, and Non-Represented employees. Check to see if your current dentist is in-network with SDC at www.superiordental.com/findDentist.asp.
