Elevator Safety Tests
The links to all Safety Tests are listed below. Each test form applies to specific test requirements. Please note
that the Annual/5-year Hydraulic Safety Test form applies to all hydraulic elevators including roped hydraulic styles.
These forms must be filled out completely for acceptance. Be certain the elevator number and addresses are
correct. Incomplete forms will be returned for correction.
Elevators may be subject to being sealed out of service if the Safety Test form is not received within 30 days of
completion of the test.
