This form is designed to provide an evaluation of the polices and practices of the company relating to the extension of Equal Employment Opportunity to all persons without regard to race, religion, color, sex, national or ethnic origin, age, handicap, or Vietnam military services. It also provides the city with the race and gender make up of the business owner(s).
