Individual Tax Forms:

• 2008 Individual Declaration of Estimated Cincinnati Income Tax and Payment Coupons - form used by all individual taxpayers to estimate their tax liability for the current year. Also, coupons used to remit quarterly estimated (net profit) tax payments.

• 2008 Quarterly Withholding Tax Forms (Forms Q-1 through Q-4) - coupons used by employers to remit quarterly withholding tax payments.
  
  
• 2008 Monthly Withholding Tax Forms (Forms M-1 through M-12) - coupons used by employers to remit monthly withholding tax payments.

General Forms:

• ACH Credit Registration Form - form used to register employers for the ACH Credit Electronic Filing Program, which permits them to file their withholding payments electronically. To file using the ACH Credit Program, please contact your bank to ensure that they can process ACH credit transactions. If your bank does offer this service, ask for their ACH transmission schedule. • Cincinnati Extension Request - used by all taxpayers to request an extension of time (maximum of six months) for the purpose of filing their City of Cincinnati Income Tax Return.
  
  
• New Account Application Form - the worksheet completed by all taxpayers to establish a new City of Cincinnati tax account.
  
  
• Taxpayer Information Sheet - the worksheet used to document residential addresses and employment history of taxpayers.
