Please use the form to report incidents for complaints against Cincinnati police officers, fraud, waste, abuse, and ethical misconduct by employees of the City of Cincinnati.
This form is only meant to be used to report allegations against Cincinnati police officers, fraud, waste, abuse or misconduct on the part of employees of the City of Cincinnati. It is not meant to be used to report issues such as sexual harassment, equal employment opportunity related issues, or other grievances better reported to Human Resources.
Please be aware that any investigations that come out of tips can only be as good as the information provided. Be as complete as possible with any information you have. If you would like feedback on a complaint, but wish to remain anonymous there are free email services available on the Internet that you could use to communicate with CCIA.
Other Options To Report A Citizen Complaint
- In Person: Come to 805 Central Ave., Suite 610, Monday through Friday from 8 a.m. to 4:30 p.m., or call CCIA at 513-352-1600. The staff can respond to questions and submit a complaint form for you.
- Email: Submit your complaint to CCAfirstname.lastname@example.org. Be sure to include your name, phone number, and mailing address.
